For attorneys and medical-legal matters
Attorneys and firms can use this form for expert witness work, independent medical examinations, fitness for duty questions and records review. Please leave patient health information out of it. Dr. Weiss will take what he needs on the call.
Phone hours are Monday – Friday 10 a.m. – 4 p.m.
Our office is closed on major holidays and during inclement weather per AISD status.
